Before admitting any dog to our facility, owners must complete the registration form and waiver that confirms up-to-date shots (with proof from your veterinarian), temperament and health history. Please download both of the attached forms and either bring them to Cheakamus Doggie Den in person, or fax to us toll-free at 1-888-535-9128.

Dogs with a history of aggressive behaviour toward other dogs or humans are not permitted. We kindly ask all clients to call in advance regarding potentially aggressive breeds.* We take your dog's safety seriously and cannot make exceptions under any circumstance. * Please be advised that we do not accept Pit Bull Terriers or Staffordshire Terriers.
